How to Send FormCraft Submissions to Tebra (Kareo)

Tebra (Kareo) is a healthcare platform. Here is how to pipe FormCraft submissions straight into Tebra (Kareo) using Advanced Form Integration. The setup takes about five minutes and runs entirely on your own server.

The direct approach

What you will need

  • A WordPress site with FormCraft installed and at least one form.
  • A Tebra (Kareo) account.

Connect Tebra (Kareo) to AFI

Tebra (Kareo) uses OAuth 2.0 for authentication. In AFI → Settings → Tebra (Kareo), click Authorize and grant access. This is a one-time step.

Setting up the integration

1. Create a new integration

Go to AFI → Add New. Name it FormCraft to Tebra (Kareo).

2. Pick your trigger

Select FormCraft as the form provider and choose the form you want to connect. AFI detects the fields automatically.

3. Select Tebra (Kareo) as the destination

Choose Tebra (Kareo) from the platform dropdown and pick Add Patient. This creates a patient in Tebra.

4. Map fields

Match each Tebra (Kareo) field on the left to the corresponding FormCraft field on the right. You can also use smart tags like {{_submission_date}} and {{utm_source}}.

5. Add conditions if needed

Only want to send certain submissions? Set up a conditional rule based on any form field value.

6. Save and test

Click Save Integration. Fill out your FormCraft form and submit. Check Tebra (Kareo) to confirm the data arrived. If anything looks wrong, open AFI → Log to see the exact request and response.
