How to Send Quill Forms Data to Dynamics 365 Field Service

Connect Dynamics 365 Field Service to AFI

Setting up the integration

1. Create a new integration

Go to AFI → Add New. Name it Quill Forms to Dynamics 365 Field Service.

2. Pick your trigger

Select Quill Forms as the form provider. AFI fires when a conversational form is completed, giving you access to respondent name, email, phone, and any custom form answers.

3. Select Dynamics 365 Field Service as the destination

Choose Dynamics 365 Field Service from the platform dropdown and pick Create Work Order. This creates a work order in Dynamics 365 Field Service.

4. Map fields

Match each Dynamics 365 Field Service field on the left to the corresponding Quill Forms field on the right. Common mappings include respondent name, email, phone, and any custom form answers. You can also use smart tags like {{_submission_date}}.

5. Add conditions if needed
