How to Send Ninja Forms Submissions to DrChrono

DrChrono is a healthcare platform. Ninja Forms makes it easy to build forms, but sending data to external platforms requires an integration. AFI bridges that gap to DrChrono. Here is the full setup.

What you will need

• A WordPress site with Ninja Forms installed and at least one form.
• A DrChrono account.
• The Advanced Form Integration plugin (free from WordPress.org).

Connect DrChrono to AFI

DrChrono uses OAuth 2.0 for authentication. In AFI → Settings → DrChrono, click Authorize and grant access. This is a one-time step.

Setting up the integration

1. Create a new integration

Go to AFI → Add New. Name it Ninja Forms to DrChrono.

2. Pick your trigger

Select Ninja Forms as the form provider and choose the form you want to connect. AFI detects the fields automatically.

3. Select DrChrono as the destination

Choose DrChrono from the platform dropdown and pick Create Patient. This creates a patient in DrChrono.

4. Map fields

Match each DrChrono field on the left to the corresponding Ninja Forms field on the right. You can also use smart tags like {{_submission_date}} and {{utm_source}}.

5. Add conditions if needed

Only want to send certain submissions? Set up a conditional rule based on any form field value.

6. Save and test

Click Save Integration. Fill out your Ninja form and submit. Check DrChrono to confirm the data arrived. If anything looks wrong, open AFI → Log to see the exact request and response.
