How to Send Forminator Form Submissions to Success.ai

Success.ai is a email outreach platform for managing subscribers and campaigns. Forminator lets you build forms, polls, and quizzes, but sending data to external platforms needs an integration. AFI bridges that gap to Success.ai. Here is the full setup.

What you will need

• A WordPress site with Forminator installed and at least one form.
• A Success.ai account.
• The Advanced Form Integration plugin (free from WordPress.org).

Connect Success.ai to AFI

1. Log in to your Success.ai account.
2. Go to Settings → API and copy your API Key.
3. In WordPress, go to AFI → Settings → Success.ai.
4. Paste the API Key and click Save Changes.

Setting up the integration

1. Create a new integration

Go to AFI → Add New. Name it Forminator to Success.ai.

2. Pick your trigger

Select Forminator as the form provider and choose the form you want to connect. AFI detects the fields automatically.

3. Select Success.ai as the destination

Choose Success.ai from the platform dropdown and pick Add Lead. This adds a lead to Success.ai.

4. Map fields

Match each Success.ai field on the left to the corresponding Forminator field on the right. You can also use smart tags like {{_submission_date}} and {{utm_source}}.

5. Add conditions if needed

Only want to send certain submissions? Set up a conditional rule based on any form field value.

6. Save and test

Click Save Integration. Fill out your Forminator form and submit. Check Success.ai to confirm the data arrived. If anything looks wrong, open AFI → Log to see the exact request and response.
