How to Send Forminator Form Submissions to Practice Better

Practice Better is a health coaching platform. Forminator is a powerful form builder from WPMU DEV. Practice Better is where that data needs to go. Here is how to connect them without Zapier or code.

Why not just use Zapier?

For connecting Forminator to health coaching, a cloud automation service adds cost and a dependency on an external server. AFI runs on your site with no task limits.

What you will need

• A WordPress site with Forminator installed and at least one form.
• A Practice Better account.
• The Advanced Form Integration plugin (free from WordPress.org).

Connect Practice Better to AFI

Practice Better uses OAuth 2.0 for authentication. In AFI → Settings → Practice Better, click Authorize and grant access. This is a one-time step.

Setting up the integration

1. Create a new integration

Go to AFI → Add New. Name it Forminator to Practice Better.

2. Pick your trigger

Select Forminator as the form provider and choose the form you want to connect. AFI detects the fields automatically.

3. Select Practice Better as the destination

Choose Practice Better from the platform dropdown and pick Add Client. This creates a client in Practice Better.

4. Map fields

Match each Practice Better field on the left to the corresponding Forminator field on the right. You can also use smart tags like {{_submission_date}} and {{utm_source}}.

5. Add conditions if needed

Only want to send certain submissions? Set up a conditional rule based on any form field value.

6. Save and test

Click Save Integration. Fill out your Forminator form and submit. Check Practice Better to confirm the data arrived. If anything looks wrong, open AFI → Log to see the exact request and response.
