How to Send Everest Forms Submissions to Zendesk

Zendesk is a helpdesk platform. Every Everest Forms submission should flow into Zendesk automatically. This guide shows how to set that up in five minutes.

The direct approach

What you will need

• A WordPress site with Everest Forms installed and at least one form.
• A Zendesk account.
• The Advanced Form Integration plugin (free from WordPress.org).

Connect Zendesk to AFI

Zendesk uses OAuth 2.0 for authentication. In AFI → Settings → Zendesk, click Authorize and grant access. This is a one-time step.

Setting up the integration

1. Create a new integration

Go to AFI → Add New. Name it Everest Forms to Zendesk.

2. Pick your trigger

Select Everest Forms as the form provider and choose the form you want to connect. AFI detects the fields automatically.

3. Select Zendesk as the destination

Choose Zendesk from the platform dropdown and pick Create Ticket. This creates a ticket in Zendesk.

4. Map fields

Match each Zendesk field on the left to the corresponding Everest Forms field on the right. You can also use smart tags like {{_submission_date}} and {{utm_source}}.

5. Add conditions if needed

Only want to send certain submissions? Set up a conditional rule based on any form field value.

6. Save and test

Click Save Integration. Fill out your Everest Forms form and submit. Check Zendesk to confirm the data arrived. If anything looks wrong, open AFI → Log to see the exact request and response.
