How to Send Quform Submissions to Tebra (Kareo)

Tebra (Kareo) is a healthcare platform. Every Quform submission should flow into Tebra (Kareo) automatically. This guide shows how to set that up in five minutes.

Connect Tebra (Kareo) to AFI

Tebra (Kareo) uses OAuth 2.0 for authentication. In AFI → Settings → Tebra (Kareo), click Authorize and grant access. This is a one-time step.

Setting up the integration

1. Create a new integration

Go to AFI → Add New. Name it Quform to Tebra (Kareo).

2. Pick your trigger

Select Quform as the form provider and choose the form you want to connect. AFI detects the fields automatically.

3. Select Tebra (Kareo) as the destination

Choose Tebra (Kareo) from the platform dropdown and pick Add Patient. This creates a patient in Tebra.

4. Map fields

Match each Tebra (Kareo) field on the left to the corresponding Quform field on the right. You can also use smart tags like {{_submission_date}} and {{utm_source}}.

5. Add conditions if needed

Only want to send certain submissions? Set up a conditional rule based on any form field value.

6. Save and test

Click Save Integration. Fill out your Quform form and submit. Check Tebra (Kareo) to confirm the data arrived. If anything looks wrong, open AFI → Log to see the exact request and response.
